The Electronic Medical Record (EMR) initiative in Putrajaya encompasses the deployment of Clinic Information Systems (CIS) across government health facilities to digitize patient records and streamline healthcare delivery.

The system is championed by the Ministry of Health Malaysia (Kementerian Kesihatan Malaysia) and supports the Putrajaya District Health Office (Pejabat Kesihatan Putrajaya). Currently, the implementation features two distinct systems across public health clinics:

  • FISICIEN: Operating at Klinik Kesihatan Putrajaya Presint 9 since 1999.
  • TPC-OHCIS: Operating at Klinik Kesihatan Putrajaya Presint 18 since 2015 (migrated to Teleprimary Care - Oral Health Clinic Information System in Dec 2020).

Between 2021 and 2025, these systems successfully registered 113,898 residents, with historical cumulative registrations in the FISICIEN system reaching 314,780 — reflecting long-term patient record accumulation across Putrajaya Clinic’s history since 1999.

Challenges & Future Direction: Currently, FISICIEN and TPC-OHCIS operate independently on different on-premise architectures without direct integration, meaning patients may have separate EMRs if they visit different clinics. Because developing APIs for integration incurs high costs, KKM is steering towards a unified Cloud-based Clinic Management System (CCMS) to provide seamless data interoperability across all health facilities in Putrajaya in the future.
